The Nurse Practitioner is assigned to the Home Based Primary Care (HBPC) as an interdisciplinary team member and primary medical provider, NP provides treatment to frail, chronically ill veterans in their homes. The patient population served is person with complex chronic diseases that worsen over time and provides interdisciplinary care that is longitudinal and comprehensive rather than episodic and focused.
